PRELIM EXAM FORMS & RESOURCES
All forms located on department website are to be submitted to ‘department’ graduate office. All forms located on GradSpace (Blackboard) are to be submitted to ‘The Graduate School’.

Prelim Paperwork Requirements:
· Two weeks prior to defense submit Defense Announcement Form http://www.psy.fsu.edu/grad.prog/docs/Defense_Announcement.htm Note it is not necessary to send a copy to The Graduate School because the department will send a copy. The Preliminary Doctoral Exam will not be submitted to The Graduate School.
· After the committee has been formed complete the Doctoral Supervisory Committee Form http://www.psy.fsu.edu/grad.prog/docs/Doctoral_Supervisory_Committee.htm. Clinical and Social students must submit this form by the date of dissertation prospectus. Otherwise have signed no later than Prelim Defense.

At conclusion of defense:
· Have committee sign the Defense Report Form http://www.psy.fsu.edu/grad.prog/docs/Defense_Report.htm
· Have committee sign Doctoral Program of Studies form http://www.psy.fsu.edu/grad.prog/docs/Doctoral_Program_Studies.htm (complete and attach to this form the appropriate Program Area Checklist; see below) Clinical and Social students must submit this by date of dissertation prospectus. Otherwise have signed no later than at Prelim defense.
