Notes: 1. Unless noted otherwise, all paperwork should be submitted to the department’s Graduate Office, A203G PDB. 2. Forms requiring the signature of the department chair may be signed by the Director of Graduate Studies. 3. Required information on forms that committee members need to sign at the defense should be completed by the student ahead of time.

Informational Documents

· Doctoral Degree Checklist (Psychology Department)

· University Checklist - located in the "Theses, Treatises & Dissertations" submenu, found on the left-hand side of the screen. Students should access the "GradSpace" webpage and Faculty and Staff should access the "Office of Graduate Studies - Faculty/Staff" webpage.

Note: It is not necessary to submit the university Defense Announcement form to the FSU Office of Graduate Studies; however, you must submit the departmental Defense Announcement Form to the department’s Graduate Office.

· Electronic Thesis, Treatise, and Dissertation Preparation Information can be found exclusively on the Office of Graduate Studies Blackboard websites. All ETD content and information can be located in the "Theses, Treatises & Dissertations" submenu, found on the left-hand side of the screen. Students should access the "GradSpace" webpage and Faculty and Staff should access the "Office of Graduate Studies - Faculty/Staff" webpage.

Dissertation Prospectus

            At defense

· Thesis, Treatise, Dissertation Research Approval Form - found under Theses, Treatises, and Dissertation Forms & Important Documentation in the "Theses, Treatises & Dissertations" submenu, found on the left-hand side of the screen. Students should access the "GradSpace" webpage and Faculty and Staff should access the "Office of Graduate Studies - Faculty/Staff" webpage.

Submit 1 copy to the FSU Office of Graduate Studies and 1 copy to the department’s Graduate Office.

· Final Degree Clearance Form - Thesis, Treatise, Dissertation Research Approval Form - found under Theses, Treatises, and Dissertation Forms & Important Documentation in the "Theses, Treatises & Dissertations" submenu, found on the left-hand side of the screen. Students should access the "GradSpace" webpage and Faculty and Staff should access the "Office of Graduate Studies - Faculty/Staff" webpage.

Clinical Students Only

· Independent Project Approval Form (may be approved by master's supervisory committee or doctoral supervisory committee)

· Clinical students should see Prelim Exam Forms and Resources or the department's Doctoral Degree Checklist for forms and instructions for establishing a doctoral supervisory committee.
