Fees
for Services:
Operating expenses
of the FSU Psychology Clinic (e.g., equipment, supplies, testing
kits, screening measures, staff salaries) are derived primarily
from client fees rather than state or university funding.
The application fee
required of all clients is $25. For couples the fee application
fee is $35. This fee covers the screening interview and the
scoring of all the screening measures.
The standard fee
for therapy is $60 per hour. However, we recognize that our
fees, though modest, may create a financial hardship for some
individuals. Therefore, we offer a sliding fee scale for therapy
clients, based on income and number of dependents. Sliding scale fees for therapy range from $20 to $60 an hour.
The fee for court-ordered therapy is $60 per hour. The fee for a standard court-ordered evaluation is $135. A standard court-ordered evaluation includes an intake session, MMPI-2 testing, a written report, and a feedback session.
Fees for group therapy
are $8 a session.
Fees for assessment
depend on the purpose of the evaluation and the specific tests
administered. For example, a gifted evaluation is $160 and a
full psychological evaluation (e.g., Learning Disability/Attention
Deficit Hyperactivity Disorder assessment) is $400. Questions
about the range of our testing fees can be directed to the Office
Assistants, the Office Manager, or the Assistant Director of
the FSU Psychology Clinic.
Payment
Issues:
Clients are expected
to pay at the time that services are rendered. The clinic's
ability to operate while continuing to charge the modest fees
outlined above is directly dependent upon our ability to collect
a high percentage of the fees we do charge.
The FSU clinic accepts
cash, personal checks, credit cards (Visa, Mastercard, American
Express, & Discover), and the FSU card for payment.
Medical
Insurance:
Clients are welcome to use health insurance to obtain reimbursement for services received at the clinic. However, some insurance companies will not pay for services rendered at the clinic, because the services are not provided directly by a licensed psychologist. *
If you have insurance
that provides full or partial payment of mental
health services, the clinic staff can assist you in filing for
reimbursement if you wish. However, the clinic does not accept
assignment of payment and all fees are due when services are
rendered.
When you file a claim
for insurance coverage of our services, you must also sign a
release of information form. Insurance companies often request
information regarding your diagnosis, dates of appointments,
services rendered, and charges. By using insurance coverage
and signing a release, you are giving the clinic permission
to release this information to the insurance company. Thus,
you should sign the form knowing the implications of using insurance
to pay for services.
Please note that
insurance companies will usually not pay charges for missed
appointments; clients are responsible for these fees.
The clinic does not
accept payments made by Medicare or Medicaid.
* The exception to
this is for services received by the licensed psychologist-in-residence
at the clinic, Dr. Donald Kerr, who is a provider for a number
of insurance companies including Blue Cross and Blue Shield.

Missed
appointments:
If you must miss
a scheduled appointment with your therapist, 24 hour notice
of cancellation is required. You may call your therapist directly
if you have their phone number, or you may call the clinic at
(850) 644-3006. Monday appointments can be cancelled by leaving
a message on the clinic answering service (voice mail) over
the weekend. Messages will be checked first thing Monday morning
and your cancellation will be relayed to your therapist. There
is a fee for cancelled appointments when 24 hours notice has
not been given. Therapy clients will be charged their hourly
fee and assessment clients will be charged a flat fee of $40
per missed appointment without adequate notice.
