The steps in the application process are as follows: 1) complete the appropriate application for services (adult or child/adolescent), the FSU Psychology Clinic Income Information form, and the Consent for Taping, Observation, and Research Participation form; 2) schedule a screening interview at the clinic; 3) complete a screening interview and fill out various psychological questionnaires; 4) for adult or adolescent clients, make an appointment to complete the Minnesota Multiphasic Personality Inventory-2 (MMPI-2) or Minnesota Multiphasic Personality Inventory – Adolescent version (MMPI-A), as applicable; 5) return to the clinic to complete the MMPI-2 or MMPI-A; and 6) pay the application fee. Each of these steps will be described below in turn.

Scheduling a Screening Interview

Screening interviews can be scheduled by calling the FSU Psychology Clinic at (850) 644-3006. Screening personnel have approximately 10 regular time slots a week that you can choose from for your screening interview. Most of the time, screening interviews can be scheduled for the week after your call.

Screening Interview

Please be prompt to your scheduled screening interview. You can report to the waiting room on the 2nd floor of the Regional Rehabilitation Center for your screening interview. For directions to the clinic, please see: directions to the clinic.

Please let the office assistant know that you are there for a screening interview. Upon arrival to the clinic, you will be given the application (and associated forms) to complete if you have not already filled these out in advance. You will also be given a number of psychological questionnaires to complete to give us more information about your current symptoms.

One or both legal guardians of children or adolescents seeking services at the clinic should accompany that child or adolescent to the screening interview. Both members of couples seeking therapy should attend the screening interview.

The screening interview will be conducted by a graduate student psychological trainee or the Assistant Director of the clinic. Screening interviews last approximately ½ hour. The screening interview is meant to give you information about the clinic, give you a chance to ask questions, and to get basic information from you about why you are seeking services at this time. It is also to determine whether the clinic can meet your treatment or assessment needs.

If it is determined that we can not meet your needs, you will be provided with appropriate referrals to other community resources.

Make an Appointment for the MMPI-2 or MMPI-A

After the screening interview, you can make an appointment to take the Minnesota Multiphasic Personality Inventory-2 (MMPI-2). Office assistants or screening personnel can assist you in making this appointment. Depending on the time and room availability, you may be able to complete the MMPI-2 immediately after your screening interview.

Only adult or adolescent clients have to make an appointment to take this test. Adolescents are persons between the ages of 14 and 17. Persons 18 years or older are considered to be adults. Children 13 years or younger seeking services skip this part of the application process.

Complete the MMPI-2 or MMPI-A

The MMPI-2 is a 567-item, true-false questionnaire. You must take this psychological questionnaire for your case to be assigned to a therapist. You will be placed in a private room in the clinic to take this test. The MMPI-A is comparable in length and format.

Only adult clients complete the MMPI-2. Adolescent clients complete the MMPI-A. Child clients do not have to complete either measure. Instead, the legal guardian completes the Child Behavior Checklist at the time of the screening interview.

Application Fee

There is a $25 application fee for clients applying for an assessment or individual therapy services. This fee covers the screening interview, MMPI scoring (as applicable), and other questionnaire scoring. The fee for couples or families is $35. This fee is payable at the time application to the clinic is made.

What’s Next?

After completing the above steps, you will be assigned to a therapist. Cases are usually assigned within a few weeks of completing the above process. This process may take longer if there is a waiting list for services. Your assigned therapist will contact you to set up an interview.
